You could also do this easily in FrameScript with the following two lines:
Get Object Type(CondFmt) Name('OldConditionName') NewVar(condvar);
Set condvar.Name = 'NewConditionName';
This would change the condition tag named 'OldConditionName' to
'NewConditionName'.
