Have a look at Index Tools and Index Tools Professional at
<http://www.kagi.com/SPSoftware/>.  Index Tools inserts continuation lines
and merges orphan subentries, e.g.

     dogs
          care and feeding of

becomes

     dogs, care and feeding of
